Incident Investigation

This course provides essential training on the systematic process of investigating workplace incidents, near misses, and property damage. Participants will learn how to properly secure an incident scene, preserve physical evidence, and conduct effective, objective witness interviews. The curriculum emphasizes moving beyond assigning blame to utilizing proven root cause analysis techniques (such as the "5 Whys" and Fishbone diagrams) to identify underlying systemic failures. Finally, the training covers how to develop, implement, and track corrective and preventive actions (CAPA) to ensure a safer work environment moving forward.
